Do I need to use distilled water for the initial rinse in step 1 or just in the final rinse? The instructions don’t specify which type of water I need to use for the initial rinse.
will microwaving eliminate contamination by amoebas?

No need to use the distilled water for the initial rinse.  If you use microwave with the empty bottle, cap and tube for 20 to 30 seconds, all microorganisms will not survive. Now you use the previously boiled and cooled down water or distilled water for the nasal rinse, there should not be any microorganisms of concern.  I hope this explains.
