i have this kind of nose and mouth problem since a long 7 years ago it seems me as a chronicle disease for me because i had consulted so many ENT doctors to rectify my problem and lastly i have take the surgery for nose also even the problem -the problem is that i am very suffering to speak fluent and for take breath...

Try and see a lung specialist and check on your lung capacity and diaphragms. May need to see a neurologist for the neuromuscular strength. You need to be with a doctor who can coordinate with other specialists and possibly resolve the case. Too complicated situation to deal with on the internet. Please make sure to stay in a close contact with your doctors.
