I have had surgery twice on my left sinus due to sinusitis. When I rinse that sinus I have started to hold my nose shut and shake my head up and down before I let the solution run out. Is this unnecessary or even harmful?

It is not harmful to do this and in your case may even help to dislodge more mucus.
