This is a unique and tricky situation. You might try buying a baby bottle nipple and inserting it over the Sinus Rinse round cap or neti pot spout. Then cut the nipple to fit in your nostril just past the perforation. The trick is to push the saline solution beyond the point of the perforation. Please let us know if you try this and it works.
