The instruction booklet that came with my Sinus Rinse says to heat the distilled water in the microwave but it doesnt say how wam it should be. Also - does it have to be warmed?

The distilled (or filtered [through 0.2 micron], or previously boiled) water is more comfortable for rinsing when it is at lukewarm or body temperature. You do not have to warm it if you do not wish to.
